Liverpool City Council Adult’s Services

Placements will continue to be organised by the university Placement Coordinator.

Liverpool City Council offers a variety of placement opportunities which we hope prove to be a positive and rewarding experience which will enable students to build upon the academic learning and teaching they receive as part of their degree course.

Localities

The 12 locality teams are split into North Locality and South Locality are based across three sites. To undertake strength and asset-based Care Act Assessments/Reviews, support planning, setting personal outcomes and provision of services which support people to return/continue to live independently within their own community.

Adult Access

Adult Access includes the safeguarding triage and multidisciplinary team (MDT), they operate core hours. The safeguarding triage team will screen and triage all safeguarding referrals and send those that meet the criteria for an enquiry under s42 of Care Act to Locality teams.

Hospital Discharge Team

Experience in complex case management, Care Act Assessments, Mental Capacity Assessments and a wide range of other tasks that will help achieve a safe and timely discharge.

Care Home Team

Complete case management for the residents within homes funded by LCC and provide support to the care home to increase partnership working. Safeguarding investigations for residents of homes across Liverpool, out of city cases and legal work such as COP applications and joint funding applications.

Community Mental Health Teams (CMHT)

Assessment and care planning for adults with mental health needs: Care Act 2014, Mental Capacity Act 2005 and Mental Health Act 1983.
